The P.I.E.C.E.S. Approach

P.I.E.C.E.S.™ is a holistic, person, and care partner centred collaborative approach which enhances capacity at the individual, TEAM, organization and system levels to support older adults who are living with complex chronic conditions, including neurocognitive disorders (including but not limited to the dementias) and/or mental health needs and associated behavioural changes.

The P.I.E.C.E.S.™ approach provides a unique way of understanding the person’s multiple health challenges and associated risks, promoting enhanced quality of life by recognizing individual needs and building on strengths related to the person’s Physical, Intellectual and Emotional health, supportive strategies to maximize Capabilities, the individual’s social and physical Environment and Social self (cultural, spiritual, Life Story).

The P.I.E.C.E.S.™ Learning and Development Program provides health care practitioners across the continuum of care with a practical, reflective and evidence based approach to shared assessment, collaborative engagement and supportive care for older adults living with complex health challenges. It uses a shared solution finding framework to surface collective wisdom of the person, care partner and all TEAM members, focused on prevention, early detection and a continuous process for shared care, monitoring progress and minimizing disability. This framework complements and integrates other strategies, approaches and relevant bodies of knowledge. By providing a common set of values, a common language and a common yet comprehensive approach to collaboration, P.I.E.C.E.S. supports a shared accountability for person and care partner-directed care across the system of care; with an emphasis on health promotion, prevention, and chronic disease management.

Targeted learners are Regulated Health Professionals or those health care practitioners who have a clinical responsibility and accountability for shared assessment and shared care planning, who are interested in becoming a P.I.E.C.E.S. Resource Person within their organization. Developing the Core Competencies will require support from the organization’s senior leadership team and external partners such as the local Psychogeriatric Resource Consultant /Seniors Mental Health services/and other partners in care; this will improve TEAM and individual practices, and promote collaborative care.

Please note:  PSW's and other unregulated front line employees are not eligible to attend the P.I.E.C.E.S. Program.  Please consider the U-First! Program for these employees.

Core Competencies

The P.I.E.C.E.S. learning & development program focuses on the development of six core competencies to:
+ Detect or flag neurocognitive/mental health needs and associated behavioural issues.
+ Use the P.I.E.C.E.S. shared solution finding Template to guide a systematic, comprehensive, TEAM approach to complex issues.
+ Use the recommended tools to collect data.
+ Plan care with others (internal and external to the organization).
+ Evaluate based on the goals developed through shared care planning.
+ Coach other staff to develop the above five competencies in others

Cancellation and Refund Policy

We appreciate that unusual circumstances and emergencies arise on education days; we deal with these on a case-by-case basis. To accommodate those on the wait lists, the following cancellation/refund policy applies to all registrants:

  1. You may withdraw your paid registration prior to Day 1 and you will receive a refund less the Eventbrite fees by contacting us by email: piecesoffice@execulink.com .  If you provide a replacement no refund will be issued and no additional fees will be charged by Eventbrite.
  2. If you are a “no show” you will not receive a refund.
  3. If you register for a session outside the region where you work, or you do not meet the eligibility criteria, you will not be accepted into the program.  A refund will be issued less the Eventbrite fees.
  4. A full refund will be issued for a session that is cancelled.
  5. Anytime a refund is requested, other than for a cancelled session, the Eventbrite fees will not be refunded.
