Dinner following presentation and Q & A

Program details:

With the urgent need for a national strategy on seniors care, BC can learn from the proven model of On Lok’s program in San Francisco as a successful approach to supporting frail elderly to age at home. Keynote Speaker Dr Catherine Eng, Medical Director of On Lok Lifeways, will give a presentation on this innovative model, today known as Program of All-inclusive Care for the Elderly (PACE) that has been replicated across the US providing high quality, cost-effective integrated care for high-risk and frail senior populations.

On Lok SeniorHealth is a comprehensive health plan that provides long-term care for eligible seniors living in 3 counties around the San Francisco Bay Area: San Francisco, Alameda, and Santa Clara. The program offers full medical care and support services with the goal of helping seniors live at home and in the community for as long as possible.
