Advance Care Planning Workshop
Event Information
About this event
Nearly all Canadians agree that talking about your wishes for health care and treatment is important. Yet, fewer than 1 in 5 people have done any Advance Care Planning.
Advance Care Planning (ACP) is a normal part of the life journey. It starts with a conversation with your family and loved ones. Yet, many Canadians tell us that they would be more comfortable talking about Advance Care Planning if they had the support of health professionals or other professionals. This workshop is a practical way to provide that support.
Goals of the ACP Workshop
After participating in the workshop, participants will be more:
- Knowledgeable about ACP and understand that it is a lifelong process adults can engage in at any age or stage of life.
- Aware of the tools and resources that can help them with Advance Care Planning.
- Ready to engage in Advance Care Planning.
- Comfortable and confident talking to their family, friends and health care professionals about their wishes for their care.
Facilitated by End-of-Life Planning Facilitators Michael Williams and Diane DeVivo
