About this event

**In light of COVID19 we are going to continue the education sessions online via Zoom (secured).**

The Comprehensive Advanced Palliative Care Education (CAPCE) program combines the “art” and “science” of Hospice Palliative Care (HPC). The program embeds best practice standards and aligns with the Model to Guide Hospice Palliative Care. CAPCE focuses on the development and role of the nurse as a HPC resource for the inter-disciplinary team in long-term care homes, agencies, hospitals and communities.

Dates and Time:

CAPCE runs on Thursdays from 4:30pm – 8:30pm

Fall: Thurs, September 9th , 2021 - Thurs, Jan 27th, 2022 (20 weeks)

***Please note*** RPNs, RNs, NPs, and other advanced care clinicians wishing to take the CAPCE course are required to complete, free of charge, a two-week Enhanced Fundamentals course after the 6-week basic Fundamentals course.

CAPCE can be completed in a minimum of 20weeks. Fundamentals Enhanced is provided within the first 2 weeks of the CAPCE course.

The components of the program include: self- directed reading and reflection, 6 e-learning modules, 3 in-person sessions (case based learning), 8 peer to peer exchanges (learning debrief), 2 practical application assignments and 9 individual or small group coaching sessions.

CAPCE Selection Criteria: a minimum of 1 year hospice palliative care nursing experience, have access to an internet-enabled computer, have knowledge of basic computer programs, and have completed the Fundamentals Core and Enhanced Hospice Palliative Care Programs.

**Please bring your Fundamentals Textbook for the 1st class** Resources must be picked up in person prior to joining class.

Limit 15 participants per class. First come first serve! Payment is non-refundable or transferable.

Organizer of Comprehensive Advanced Palliative Care Education

Originally founded as West Hill Community Services in 1977, Scarborough Centre for Healthy Communities (SCHC) is one of the most established and comprehensive community health organizations in the province. The organization owes its beginnings to 13 community agencies, a core group of community leaders and countless volunteers who identified emerging health and social needs within Scarborough.

After incorporating and acquiring not-for-profit status, SCHC built on the initial services of our Meals-on-Wheels, home support, transportation and family support programs.  After a successful rebranding campaign, we employ over 155 staff, engage over 650 volunteers and operate 38 distinct and integrated services across 11 sites that work together to improve the health of our community.  We provide medical assistance through our clinics, are involved in a growing youth program and have a multitude of social support programs including a food bank.
