Yorktown Family Services - Connect4 Youth Mentorship Program
Are you interested in hanging out with other youth through a group mentorship lens? To express yourself and engage in positive youth development?
Who? Holland Bloorview youth with disabilities between the ages of 12-25, who are able to focus on a task for at least 30 minutes at a time, online.
What? A virtual Transition Pop-Up event. A monthly drop-in Life Skills program empowered by youth! Join us for multimedia discussions on biases and stereotypes around disabilities, language and how we communicate, how to physically and verbally support our peers, and more! A safe space for youth to express and develop their identity through a group mentorship lens. Opportunities to community involvement, group activities, hear from Youth Mentors, and meet other youth! Topics of discussion vary each month.

4:00pm - 5:00pm
Why? You'll have an opportunity to meet other young adults from Yorktown Family Services! This program is designed to allow youth to express and develop their identity through a group mentorship lens. The Pop-Up event promotes positive youth development and engagement in a supportive, safe and creative way by connecting youth with mentors and the Youth Mentorship Team at Yorktown Family Services. Activities focus on fostering and building social skills and life skills while creating a platform for youth to explore goals that are important to them in their day to day lives.
Please note:
- This Transition Pop-Up event is a monthly drop-in session to Yorktown Family Services' weekly, Connect4 Youth Mentorship Program
- A referral will be made to Yorktown Family Services providing a warm handover to their programs and services. You will remain as a client receiving services from Holland Bloorview as well.
- Yorktown Family Services will follow up with the youth & family after the event to see if the program was a good fit for the youth moving forward; with an opportunity to participate in ongoing programming.
- You are welcome to attend the monthly drop-in sessions with Transition Pop-Ups or weekly sessions with Yorktown Family Services.
- Yorktown’s Connect4 weekly Youth Mentorship Program includes topics around goal setting related to education, employment, community engagement, engaging with other youth, and more!
