8 out of 10 people say it is important to put their health care wishes in writing, but only 2 out of 10 have actually done so.

Find out how to make your health care wishes known by attending this FREE session facilitated by AHS Advance Care Planning Educators.

If you were unwell and unable to speak for yourself, do those you are close to know your wishes for health care and personal decisions? Advance Care Planning is the process of thinking about, talking about and documenting your wishes for health care if you are unable to speak for yourself. This session will provide information on Advance Care Planning and the role of Personal Directives.
