Coordinated Care Planning - 101

Multiple providers, appointments and complex care issues can make it difficult to meet an individual’s unique needs. A more collaborative approach to providing care can be achieved through a process called Coordinated Care Planning (CCP). This process is about bringing multiple providers together, with the individual and his/her informal supports, to collectively understand the goals of the individual and develop a care plan to best support him/her. Patient/client care coordination is foundational to the Ontario Health Team model and leverages a standardized data set that allows for evidence-informed, patient-centred goal setting and care planning that can be shared across sectors.

CCPs encourage a wholistic approach for individuals whose care is not well managed. It can include:

  • 4+ chronic conditions
  • Mental Health and Addictions
  • Frail Seniors
  • Those with a palliative diagnosis
  • Individuals with multiple ER visits or preventable hospital admissions.

This approach should also consider individuals challenged by

  • Low income; and
  • Social determinants (e.g. challenges with housing, social isolation, language, etc).
