All of us are complex systems, no patient is simple or a single diagnosis. Many patients often have multiple comorbidities that we may need to plan for and adapt our intended plan. This presentation will discuss some commonly (and less commonly) seen conditions that may impact therapeutic choices that may be used to treat other common conditions.

Serotonin Syndrome & Neuroleptic Malignant Syndrome: Same, Same…..But Different

Serotonin syndrome and neuroleptic malignant syndrome are rare psychiatric emergencies that share some underlying pathophysiology and often have overlapping clinical presentations, which makes prompt recognition and accurate diagnosis quite challenging. Adding to the complexity, common psychotropic combination regimens make it even more difficult to differentiate. Prompt intervention that includes cessation of contributing medications is crucial to prevent serious sequelae. However, because mood, anxiety and psychotic disorders can be chronic illnesses, complete avoidance of serotonergic or dopamine antagonists in the future is unlikely to be practical, especially in those with more refractory illness or comorbidities. Therefore, having an improved understanding of these two syndromes may not only help with the acute management process but may also further guide treatment strategies thereafter.

1. Identify potential drug-disease state interactions and assess the severity of those interactions
2. Describe strategies for managing drug-disease interactions
3. Compare and contrast serotonin syndrome and neuroleptic malignant syndrome with respect to onset of symptoms, key diagnostic criteria, management strategies, and symptom course
4. Appreciate the limitations of drug interaction software or references on drugs implicated in these syndromes
