The P.I.E.C.E.S. Approach

P.I.E.C.E.S.™ is a holistic, person, and care partner directed model which enhances capacity at the individual, TEAM, organization and system levels to support the care of the older individual living with complex chronic disorders, including cognitive and/or mental health needs and associated responsive behaviours.
The P.I.E.C.E.S.™ model provides a shared understanding of the often multiple underlying causes of behavioural expression and associated risks, recognizing areas of need, building on the person's remaining strengths, and considering the person's Physical, Intellectual and Emotional health, supportive strategies to maximize Capabilities, the individual’s social and physical Environment, and his/her Social self (cultural, spiritual, Life Story).
The P.I.E.C.E.S.™ learning and development program provides a practical framework for assessment and supportive care strategies using a comprehensive inter-professional person-directed approach. This framework complements and integrates other strategies, approaches and relevant bodies of knowledge. A common set of values, a common language for communication across the system and a common yet comprehensive approach to collaborative care are embedded within the model. This supports a shared accountability for person and care partner-directed care across the system of care focused on health promotion, prevention, and chronic disease management.
The target audience is Regulated Health Professionals or those health professionals who have a clinical responsibility and accountability for assessment and care planning who are interested in becoming a P.I.E.C.E.S. Resource Person within their organization. Developing the core competencies will require support from the organization’s senior leadership team and the local Challenging Behavior Resource Consultant / geriatric outreach/specialty team; this will improve team and individual practices and promote collaborative care.

Core Competencies

The P.I.E.C.E.S. 24hr program focuses on the development of six core competencies to: + Detect or flag cognitive/mental health needs and associated behavioural issues. + Use the template to guide a systematic, comprehensive, and team approach to complex issues. + Use the recommended tools to collect data. + Plan care with others (internal and external to the organization). + Evaluate based on the goals developed through care planning. + Coach other staff to develop the above five competencies in others
