About this event

VACCINATIONS ARE BY APPOINTMENT as well as WALK IN.

We will be administering the vaccine as follows:

Moderna 30+ years old

Pfizer 12+ years old

Bookings will be for FIRST, SECOND or BOOSTER doses of COVID-19 Vaccination.

PLEASE BRING YOUR EVENTBRITE TICKET (if pre-registered) AND HEALTH CARD TO THE EVENT

Stouffville Medical Centre and York Region Public Health have come together to provide you with a COVID-19 Vaccination Clinic for our community. Register for your COVID-19 vaccine today! If there are currently no options available, please continue to check back as additional dates are added.

Booster Dose – 12 Years of Age and Older 18 years of age and older

Available at least three months (84 days) after receipt of the second dose and for First Nations, Inuit and Metis and non-indigenous household members aged 12 and older; individuals must be 18 years old at time of receiving booster dose

This includes anyone that has received a complete series of viral vector vaccine (e.g., two doses of AstraZeneca vaccine or one dose of Janssen vaccine), anyone that has received two or three doses of a vaccine not approved by Health Canada, or a combination of any of these vaccines and an approved mRNA vaccine.

12 to 17 years of age

As of Friday, February 18, 2022, youth 12 to 17 can receive their booster dose approximately six months (168 days) after a second dose. Individuals must be 12 years old at the time of receiving the booster dose

First Nations, Inuit and Metis and non-indigenous household members age 12 and older can receive a booster dose at least three months (84 days) after receipt of a second dose

INOTE: Please dress in a manner that allows for easy access to your upper arm for vaccination (eg. T-shirts or other short sleeved tops are recommended; please avoid long-sleeved shirts, pullover sweaters or button up shirts). Dress in layers as needed.

COVID-19 Vaccination Clinic Terms & Conditions

In registering for an Immunization Clinic, you agree to the following terms and conditions:

Previous Adverse Reactions

Individuals who have had an adverse reaction to a vaccination should NOT register for this event without first speaking with their healthcare professional. Individuals who have experienced severe allergy or anaphylaxis following a COVID-19 vaccination, must have written clearance from an allergist or immunologist.

Examples of adverse reactions include, but are not limited to:

• Fainting or Dizziness

• Shortness of Breath, Chest Tightness or Wheezing

• Nausea / Vomiting

• Swelling of the Lips, Tongue or Throat

• Other Severe Allergy or Anaphylaxis

Patient Attestation

INDIVIDUALS WHO ARE UNWELL IN ANY WAY SHOULD NOT ATTEND THE BOOSTER VACCINE CLINIC. Specifically, individuals should NOT attend if they are experiencing any of the following symptoms:

1. Fever

2. Cough

3. Shortness of breath

4. Sore throat

5. Runny nose in the absence of seasonal allergies

6. Difficulty swallowing

7. New smell or taste disorder(s)

8. Nausea / vomiting

9. Diarrhea

10. Abdominal pain

11. Unexplained fatigue / malaise

12. Chills

13. Headaches that are new and persistent, unusual, unexplained or long lasting

14. Conjunctivitis (Pink Eye)

15. Muscle aches and pains

16. Decreased or lack of appetite

17. New or unusual exacerbation of chronic condition

If you are experiencing any of the above symptoms on the day of your vaccination DO NOT ATTEND and instead reschedule for a later date.

Monitoring for Adverse Reactions

All individuals attending the event are recommended to remain on site for a minimum of 5-15 minutes following receipt of their vaccination in order to monitor for adverse reactions. Seating will be made available to accommodate for this waiting period. Participants are asked to raise their hand and call for help, at which point a physician or other health care professional will be available for assessment.

Booking Considerations

COVID-19 vaccinations will be offered in the priority sequence as outlined by York Region Public Health.

A booster dose of COVID-19 vaccine is available at least three months (84 days) after receipt of the second dose for the following eligible groups:

York Region residents 18 years of age and older

Vulnerable older adults in congregate living settings

Health Care Workers and Designated Essential Caregivers as identified by the Ministry of Health

First Nations, Inuit and Metis and non-indigenous household members age 16 and older

Those who received a complete series of viral vector vaccine (e.g., two doses of AstraZeneca vaccine or one dose of Janssen (Johnson & Johnson) vaccine)

Those who received two doses of a vaccine not approved by Health Canada (see Vaccinated outside of Ontario information below)

A third dose of COVID-19 vaccine is available for moderately to severely-immunocompromised individuals, as identified by the Ministry of Health, at an interval of two months (56 days), with a minimum interval of 28 days, after the last dose of the initial primary series.

To receive a third dose, eligible residents must present a completed form or letter indicating their eligibility from their specialist or hospital program. Prescriptions for immunosuppressant medications as identified by the Ministry of Health can also be presented for third doses as needed.
