This half-day of interactive training will provide participants the opportunity to expand their knowledge and comfort level when applying the Coordinated Care Plan process.

Keynote speaker, Ms. Penny Paucha, will explore the neuroscience behind change management and help faciltate the interactive learning session. Ms. Penny Paucha has extensive experience as a facilitator, change management expert and leadership coach with physicians and healthcare leaders throughout Ontario. She is an accredited coach with the Neuroleadership Institute and utilizes her expertise in neuroscience to create evidence-based coaching and leadership programs such as the Ontario College of Family Physicians’ Leadership Development Program. Her success in facilitating and coaching leaders on the implementation of Health Links around the province resulted in being chosen as a faculty member of the IDEAS Program at the Institute of Health Policy, Management and Evaluation at the University of Toronto.

  • Individuals that attended the Dec. 14, 2017 Leading the Health Links Approach to Coordinated Care Planning training day.

  • Primary Care Providers, Nurse Practitioners, Allied Health Professionals, Hospitals, Social Service Providers, Community Service Providers, Mental Health and Addiction Providers.
