You are invited to attend a follow-up Governor session to the last Governor Quality Symposium. At this session, we will create a process which would result in the development of a collaborative Quality improvement plan by sub-region.
Based on the assessed interest from the break out groups in Guelph-Puslinch, one area of focus for each sub-region has been selected.
Improved transitions and coordinated care for Mental Health and Addictions
Health Quality Ontario (HQO) and the WWLHIN continues to be excited to launch a new simple “cross organizational collaborative quality improvent plan.” As Lee Fairclough, VP HQO, explained Waterloo Wellington has been selected to be the first LHIN provincially to use this tool to drive sub-region integration. The Waterloo Wellington LHIN is recognized as having governors ready to lead and committed to quality.
