Description

The interCultural Online Health Network (iCON) is based in UBC Faculty of Medicine’s Digital Emergency Medicine, and is a community-driven health promotion initiative that supports multicultural communities, patients, and caregivers in British Columbia to optimize chronic disease self-management.

This year marks iCON’s 10-year anniversary. Celebrating 10 years of service and partnership with the Chinese community, iCON is preparing to host a free Chinese health forum expanding on the theme of “Healthy@Home: Living Well with Chronic Conditions”.

Patients, families, health professionals and community organizations will join this event to share information and educate about early detection, as well as prevention of complications related to diabetes, high blood pressure and dementia. Exploring the theme of healthy aging, our panel of local health experts will share strategies for self-management, healthy living, and navigating community support services. During the open house session participants can take part in health screenings and, through information booths, learn about chronic disease management and resources for patients and families provided by health and community organizations.

This event will be presented in Cantonese, with simultaneous interpretation to Mandarin and English.
