For 11 years, iCON (interCultural Online Health Network, www.iCONproject.org) has been supporting multicultural communities, patients and caregivers across BC to optimize chronic disease prevention and self-management through community education, knowledge sharing, and technology. iCON is preparing to host a free Chinese health forum expanding on the theme of Healthy@Home: Living Well with Chronic Conditions (Part II): Arthritis and Osteoporosis.

十一年來，「安康」健康網絡（ww.iconproject.org）致力於支持本地多元文化社區，患者和照顧者。通過社區教育，知識共享，以及科技，「安康」希望能加強慢性病的預防以及自我管理。「安康」健康網絡是卑詩大學電子醫學急救部與卑詩省「患者為伴」計劃的合作夥伴。「安康」計畫舉辦免費的中文健康講座，主題為“家居健康，與慢性病相處之道（二）- 關節炎和骨質疏鬆”。

This event, will focus on helping patients and caregivers prevent and manage chronic conditions to achieve optimal health at home. Healthy@Home is a strategic health promotion campaign that promotes primary and secondary disease prevention seeking to keep patients, especially seniors, at home, healthy and independent. Patients, families, health professionals and community organizations will join this event to share information and educate about falls, chronic muscle and joint pain, access to nutritious food, depression and social isolation. Exploring the theme of healthy aging, our panel of local health experts will share strategies for self-management, healthy living, and navigating community support services.
