Mid-West Toronto Sub-Region - Initial Quality Improvement Initial Session

Thank you for your interest in the Quality Improvement Core Team.

In our last Local Collaborative meetings we introduced to you the individuals within your respective sub-regions that would be forming a core project team to lead quality improvement efforts within your sub-region’s initial neighbourhood-based opportunity area.

The Toronto Central LHIN has funded admission for these core project teams from each sub-region to attend the IDEAS Advanced Learning Program (ALP).

Each core project team consists of four- ALP participants and one to two executive sponsors. The core project teams from each sub region have already been selected and improvement work with the IDEAS team of quality improvement experts has started. The goal is that this initial quality improvement project will become a catalyst for ongoing improvement work in each sub-region.

In order to ensure success, each of the core project teams will need to be supported by a larger improvement team consisting of local leaders and providers from across disciplines and sectors. The core project teams will inform, consult with and use the knowledge, skills, experiences, and perspectives of the larger supporting improvement teams to develop and implement ideas on the local quality improvement project.

As part of this team, members will assist the core project team in refining opportunities, designing and testing solutions.
