Description

Join us for the upcoming Medical Psychiatry Collaborative Care Certificate (MP3C) Foundational Modules!

Saturday, February 23, 2019 (9:00am - 4:00pm)

  • Bridging the Gap Between Physical and Mental Health: The assessment and management of co-occurring physical and mental health issues remains a challenge in our healthcare system. Individual clinicians and healthcare teams are faced with increasing complexity and physical and mental health co-morbidity is a common clinical issue in daily practice. The following MP3C module will provide clinicians with practice and evidence-based approaches to identifying, engaging and managing co-occurring physical and mental health issues. Participants will develop an integrated approach to care for these patient issues and will obtain access to practice-ready toolkits to implement in their setting.

  • Patients and Families as Partners in Integrated Collaborative Care: This foundational module explores innovative, productive and practical approaches to involving patients and families in integrated collaborative care for people with mental and physical health needs. The module will create a unique opportunity to reflect on your practice and improve the engagement and activation of patients and families to be partners in their care. Strategies for collaboratively co-creating care plans and enhancing the therapeutic alliance will be examined. There will be a particular focus on the role of empathy and emotional self-regulations in creating productive partnerships.

Saturday, March 23, 2019 (9:00am - 4:00pm)

  • Evidence and Models of Integrated Collaborative Care: This foundational module will review the current evidence for integrated collaborative care, focusing on co-existing physical and mental health needs. Local models of integrated collaborative care will also be examined. Participants will gain skills, tools and resources to propose a feasible model for implementation in their own practice setting.

  • Quality Improvement in an Integrated Mental and Physical Health Setting: This module will provide participants with an introduction to quality improvement methods and tools, including the Model for Improvement, Plan-Do-Study-Act (PDSA), Fishbone Diagrams, and Process Mapping. Participants will have the opportunity to apply the methods and tools learned by participating in a large group exercise and a case study that integrates mental and physical health. The goal is that participants will be able to apply the methods and tools in their own collaborative practice settings, by identifying opportunities for improvement in integrated mental and physical health and implementing small tests of change.

Who Should Register?

The MP3C modules are designed for all healthcare providers (i.e. nurses, physicians, social workers, occupational and physical therapists, primary care, etc.), as well as managers and administrators in primary care, hospital, long-term care and community care settings who are working with patients and families to address complex care needs, with a particular focus on co-existing mental and physical needs.

About the Medical Psychiatry Collaborative Care Certificate

As part of a new approach to the education of health care providers, the Medical Psychiatry Alliance (MPA) is offering the Medical Psychiatry Collaborative Care Certificate (MP3C), a continuing education program to support learning in integrated mental and physical collaborative health care. The program aims to provide education resources for health care providers who would like to be better equipped to assess and manage patients with co-existing mental and physical health needs.

The assessment, management and prevention of integrated mental and physical health remains a challenge in our health care system. Health care providers and their teams are faced with increasing complexity of patients with co-existing mental and physical health needs as a common, clinical issue in daily practice. To address these needs, the MP3C will provide innovative and accessible education for practicing health care providers across health care sectors and patient populations.
