OPTIONS for CARE
This series of workshops is for family care partners of persons with Alzheimer’s disease or a related dementia. Care partners may sign up for the AFTERNOON sessions they feel are most relevant to them.
Part 1. Tuesday April 4 1:30pm - 3:30 pm
Community Care Access Centre and Community Service Providers: (Adult Day Program, Community Care Durham Services):
The Community Care Access Centre (CCAC) is the local point of access to community-based health care services. The CCAC was created to coordinate a variety of health services to maintain an individual’s health, independence and quality of life. Topics include home support, accessing adult day programs, hospital aftercare. Also hear about other key community services that will help you and your family member.
Part 2. Tuesday April 11 1:30pm - 3:30 pm
Accessing a Long-Term Care Home:
This workshop will help caregivers prepare for placement and adjust to their family member entering a long term care facility. Topics include: the application process, admission to a care facility and adjustment strategies.
No fee for sessions, however a donation to defray workshop costs is welcomed.
Are there requirements to attend the event?
These sessions are intended only for care partners i.e., family and friends of a person diagnosed with dementia. Multiple family members are welcome, however registration is required for each person.
What are my parking options getting to the event?
There is parking available on the entire lot near and around the building. Entrance to the office is beside Pita Deli.
