Welcome to the 9th Annual Primary Health Care Forum - Embracing Leadership, Engagement, and Transformation Registration
The South East LHIN's Primary Health Care Forum is a networking event geared for primary health care providers. The annual event brings together primary health care professionals working in the South East LHIN region and fosters information, knowledge, and best practices sharing within the primary health care sector and across health sectors.
The learning objectives for the 2016 Forum are:
Building leadership capacity in the primary care sector to promote participation in health care transformation activities
Creating opportunities to build stronger links in the region between health, social, community, and other services to improve patient experience and health outcomes
Stimulating discussion to promote engagement of all stakeholders, including patients, in the planning and performance of our health system
