Advance Care Planning: The Who, What, When and How of Being a Substitute Decision Maker

About this Event

As death nears, sixty percent of people will not be capable of making decisions about their medical care on their own behalf. You can ensure that your wishes about your care during an incapacitating illness and your care at the end of your life are honoured by appointing a Substitute Decision Maker (SDM). A legally appointed SDM becomes your Power of Attorney for Personal Care (POAPC).

The role of an SDM is to make medical decisions on behalf of another person when they cannot cognitively make those decisions for themselves.  It is imperative that those choosing an SDM and those agreeing to be an SDM are aware of the legislation governing the role.

Dr. Bob Kemp Hospice Executive Director Clare Freeman welcomes Advance Care Planning Advocate Sandra Andreychuk to share her knowledge of SDM including how to appoint an SDM, the duties and responsibilities of an SDM, when an SDM would be called upon to make decisions for another and how to make the right decisions for that person.
