Prior to the pandemic, our health system was not effectively serving older adults with complex needs and their caregivers. We cannot simply return to the status quo; we must do better.
Transforming Care: Supporting Older Adults Post-COVID in Ontario is a virtual conference that will be of particular interest to anyone in specialized geriatric services, Ontario Health and members of Ontario Health Teams.
Date: October 19-21, 2021
Time: 8:30 am – 12:30 pm each day
Cost: Free
Location: Online via Zoom (panels/sessions) and Slack (community/networking)
Audience: Ontario Health, Ontario Health Teams, Specialized Geriatric Service providers and planners.
This three-day event is split into themed morning sessions:
- The Need: Older Adults with Complex & Chronic Health Conditions Including Frailty
- The Innovation: Integrated Care in Action
- The Transformation: Post-COVID Support and Recovery
The conference will include presentations and conversations about community paramedicine, GeriMed Risk, virtual care, collaborations with community support services, aging at home and how to leverage evaluations into post-recovery planning. It will offer participants actionable strategies, new connections, and a path toward transformative care for older adults and their care partners.
Ticket Types: Concurrent sessions
Your ticket entitles you to the full three half-days of programming.
Please only select ONE ticket type.
There is one programming section that offers four concurrent panel sessions at 11 a.m. on Wednesday, October 20. You will be able to attend one of these four sessions, as well as all other programming during the three-day conference.
Each concurrent session is limited to 100 registrants, first-come, first-serve. Your ticket is categorized by one of the four concurrent sessions. The categories include:
- TICKET A: Integrated Care concurrent session
- TICKET B: Behavioural Supports Ontario concurrent session
- TICKET C: Adult Day Programs concurrent session
- TICKET D: Central Intake concurrent session
- TICKET E: FULL CONFERENCE Except Concurrent Session
