ABSTRACT: This presentation will share the findings from a one year pilot project where trained volunteer navigators provided support services in the home for older adults living with advanced chronic illness. Evaluation indicated that having a volunteer navigator improved older adult quality of life by making their chronic illness experience more 'livable'.
