Virtual Brief Action Planning workshop for health care/community professionals and people in helping roles , February 12th, 16th and 18th 2021 from 10:30am-12pm (Session A, B and C)
Brief Action Planning (BAP) addresses many of the barriers providers have cited to providing self-management support. It can be used routinely by both individual providers and health care teams to facilitate patient-centered goal setting, action planning and to build self-efficacy in chronic disease management and prevention.
BAP uses three sessions workshop format via Zoom Meeting, 90 minutes long per session to provide health care providers with an opportunity to learn specific, brief and efficient communication strategies and skills that can be utilized in the setting of brief medical office visits.
- Based on theories, principles and practices of Motivational Interviewing (MI) and the evidence-based literature of behaviour change, drawing specifically on self-efficacy and action planning theory and research.
Designed to acquaint the participant with the literature, theory and techniques for promoting change in health behaviour and supporting self-management
