Options for Care: Supporting Increased Needs in Dementia (3-Week Workshop)
Overview
This three-part series will focus on the supports available within Durham Region. Care partners will learn more about progression of dementia and the impact on the individual and care partner(s), how to evaluate their own caregiving needs, and explore supportive environments in the community.
Week 1: Impact of Progression & Creating a Support Plan (February 10th)
First in our 3-part series, this week will explore the changes that occur and impact of dementia progression on the person living with dementia and the care partners involved. Reflective activities will help determine what supports are right for you and your family member or friend.
Week 2: Residential Care Options & Making the Decision (February 17th)
Second in our 3-part series, this week will discuss what various residential care options are available, considerations to be made, and when it is the right time to decide.
Week 3: Applying to Long-Term Care & Transitions (February 24th)
Last in our 3-part series, this week dives deeper into reviewing the application process for long-term care, discussing how to support transitioning to new locations, and how to continue supporting a person living with dementia in long-term care.
