IN-PERSON Workshops

This free diabetes self-management education and support workshop series is for adults (18+) with diabetes. Guided by a registered dietitian/nutritionist, participants will learn about managing their diabetes and living a healthy life.

  • Wednesdays: 6 to 8 p.m.
  • Participants will receive a metroCard for two rides and a chance to win a Fitbit by attending all five sessions.

Tremont Neighborhood Health Action Center: Hosting a variety of programs and activities focused on the health and wellness of East Tremont residents. Our clinical partners also offer primary health services. The Action Center is part of the City's effort to promote health equity and reduce health disparities in the neighborhood.
