Please include you the name of your Ballroom House and list which menu options you want (and quantities, up to 6 in total) by JUNE 8th. Additional meals can be added at $20 a plate.

*This is an add-on, entry tickets must be purchased in addition to the table. You can share your table with any number of house members and guests
