Windsor Essex Community Health Centre (WE CHC) was formed as a result of the amalgamation of Teen Health and Sandwich Community Health Centres in 2009.
As a community Health Centre our Mission is to support the health and wellness of our vulnerable population in everything we do.
The locations we operate include:
Teen Health is located at 1361 Ouellette and provides Primary Care and Counselling to youth between the ages of 12 and 24 years. Individuals come to us for Physical and Mental Health (individual and group counseling is available); Eating Disorders; Substance Abuse; Parent Support (support for parents and guardians of adolescents) and Pre and Post Natal for young moms.
Street Health is located at 711 Pelissier and provides Primary Care and Supportive Services to individuals who are homeless or at risk of homelessness. Individuals come to us for Primary Health Care, Clinical Services, Foot Care, Dental Care, Chiropractic Care and Hepatitis C Support and Treatment.
Our Identification Clinic provides assistance in obtaining Ontario Health Card and potential supporting documents.
The Supportive Services we provide are system navigation to appropriate community resources, showers, hygiene products and laundry facilities are available.
Sandwich Community Health
Sandwich Community Health is located at the corner of Brock and College Avenue at 3320 College Avenue and provides Primary Care and Counselling Services to area residents who do not have a Health Care Practitioner. Individuals come to us for Primary Health Care, Counselling services, Chiropody Services (foot care), Nutrition Counselling, specific health related Support Programs for men and women and General Health Promotion Services such as Smoking Cessation and Diabetes Education.
Chronic Disease is located at 2885 Lauzon Pkwy unit 107 and provides a variety of services aimed to assist individuals to better manage their own health keeping people healthy and active and living at home. Our Diabetes Wellness team supports adults who are living with Type 1 or Type 2 Diabetes and those at risk of developing diabetes. Our Aging At Home team offers education, exercise, self management skills, ongoing support and resource navigation for individuals living with chronic diseases.
Falls Prevention Program
The Windsor Essex Community Health Centre is the Erie St. Clair LHIN lead for the Falls Prevention Program. The program is made up of a number of strategies that aim to provide: proper education, awareness, screening, assessment, intervention and prevention.
Chronic Disease Self Management Program
The Windsor Essex Community Health Centre is the Erie St. Clair LHIN lead for the Chronic Disease Self Management Program. The Program has two components: 1) Master Your Health, 2) Choices and Changes.
Participants set goals and action plans; problem solve; improve coping skills; and learn about becoming more active, eating healthier and managing difficult emotions. They also focus on: managing medications, pain and fatigue; using positive thinking; working with health teams; and making decisions about their health. Master Your Health is available throughout the Windsor- Essex, Chatham-Kent and Sarnia-Lambton region for adults living with chronic health conditions such as diabetes, arthritis, asthma, lung disease, heart disease, stroke, Multiple Sclerosis, Parkinson’s Disease, chronic pain and more.
Choices and Changes is an evidence-based program which is approved and accredited by The College of Family Physicians of Canada. Clinicians frequently express frustrations about patient behaviour and the impact of that behviour on health outcomes. Choices and Changes is a four and a half hour workshopdirected to the needs of clinicians working within the rigors of contemporary medical practice. The program provides the clinician with specific strategies that can be utilized within the highly time limited constraints of the typical office visit.
Located at 33 Princess Street Suite 401, WE CHC operates a primary care clinic for clients without a primary care provider who meet the follwing criteria: children, youth, adults and seniors that are in neeed of a safe place to recieve care and system navigation due to indentified risk factors. Pickwick Plaza (East Windsor) - 7621 Tecumseh Rd.
This site offers primary care (by appointment) to individuals who do not have a doctor or a nurse practitioner. There are several group programs offered at this site including; Diabetes Education, Active Aging Therapeutic Exercises, Master Your Health Self-Management, Tai Chi and Weight Management.
