Yes / No</strong></h3><ul><li><strong>Fever (feeling hot to the touch, a temperature of 37.8 degrees Celsius or higher)</strong></li><li><strong>Cough that&#x27;s new or worsening (continuous, more than usual)</strong></li><li><strong>Barking cough, making a whistling noise when breathing(croup) </strong></li><li><strong>Shortness of breath (out of breath, unable to breathe deeply)</strong></li><li><strong>Sore throat (not related to seasonal allergies or other known causes or conditions)</strong></li><li><strong>Runny nose (not related to seasonal allergies or other known causes or conditions)</strong></li><li><strong>Stuffy or congested nose (not related to seasonal allergies or other known causes or conditions)</strong></li><li><strong>Lost sense of taste or smell</strong></li><li><strong>Headache that’s unusual or long lasting</strong></li><li><strong>Digestive issues like nausea/vomiting, diarrhea, stomach pain (not related to other known causes or conditions)</strong></li><li><strong>Muscle aches that are unusual or long lasting</strong></li><li><strong>Extreme tiredness that is unusual</strong></li><li><strong>For young children and infants: sluggishness or lack of appetite</strong></li></ul><h3><strong>2. In the last 14 days, have you been in close physical contact (living in the same home and less than 2 metres away) with someone who either?  Yes / No</strong></h3><ul><li><strong>Is currently sick with a new cough, fever, or difficulty breathing? </strong></li><li><strong>Is waiting a test result for COVID-19?</strong></li><li><strong>Has tested positive for COVID-19?</strong></li><li><strong>Returned from outside of Canada in the last 2 weeks and has symptoms of COVID-19?</strong></li></ul><h3><strong>3. Have you travelled outside of Canada in the last 14 days?   Yes / No</strong></h3><h2>NOTE:</h2><h2>In order to protect other brothers and sisters when we come into our church for in-person services, we would highly recommend that those who plan to come in for in-person services would first receive COVID-19 vaccination.  This will ensure a safe and healthy environment for everyone who comes into our church.</h2><h2></h2>
