Aim: To introduce a Precision Social Prescribing (PSP) toolkit to address neighbourhood disparities in mental health among BC older adults, and to facilitate synergistic partnerships across health and community resources.Program: 1:00 pm - 1:30 pm <ul><li>BACKGROUND and solutions to neighbourhood disparities in mental wellbeing among older adults during a pandemic. </li></ul>1:30 pm - 2:00 pm <ul><li>INNOVATIONS 6x 5-minute rapid fire / pecha-kucha sharing on innovative propositions and experiences of Social Prescribing.</li></ul>2:00 pm - 2:20 pm <ul><li>Break</li></ul>2:20 pm - 2:50 pm <ul><li>WORKSHOPS (Concurrent options)</li><li>#1: LINKAGES across health and community sectors (Led by Fraser Health)</li><li>#2: SELF-CARE techniques to avoid burnout for frontline staff and volunteers (Led by Frog Hollow Neighbourhood House)</li><li>#3: OAKRIDGE-FAIRVIEW building community solutions (Co-led by Marpole-Oakridge Family Place and VanDusen Botanical Gardens Association)</li></ul>*All times listed are in Vancouver time (PST)*Target Audience: Medical office assistants, Pharmacists, Gerontological nurses, Social workers, Family physicians, Neighbourhood houses, Community centres, Home health, Older adults, and Decision makers.
