Getting Started: Diabetes Management

Nurse Practitioner-Led Clinics are a new primary health care delivery model in which Nurse Practitioners are the lead providers. The team at Emery Keelesdale includes Registered Nurses, Registered Practical Nurses, a Dietitian, a Social Worker, a collaborating family physician, and other health care professionals. We offer a comprehensive, accessible, and coordinated family health care service to our community - especially those who do not have access to a primary care provider. EKNPLC has been providing services to Humber Summit in North York West and the Keele & Eglinton area since 2012.

The information you provide in the Eventbrite form will be used to register you on the Abbott website so that you can obtain a <strong>free test kit</strong>. Feel free to contact us if you have any questions.</p><p></p><h3>ABOUT:</h3><p>You&#x27;re invited to attend a free, in-person education session designed to help you get started with the FreeStyle Libre 2 flash glucose monitoring system! Get a FREE test kit! Light refreshments will also be available.</p><p>This education session will be hosted by an Abbott representative alongside a diabetes educator. During the event, you will learn how to:</p><ul><li>Apply and use the FreeStyle Libre 2 system</li><li>Read your glucose scans and start to see patterns</li><li>Learn from your levels, understanding the impact of food, exercise and medication</li><li>Improve your diabetes management with simple tricks and tips</li></ul><p></p><h3>NOTE:</h3><p>(1) The FreeStyle Libre 2 app is only compatible with certain mobile devices and operating systems. Please check the website for more information about device compatibility before using the app.</p><ul><li><a href="https://www.freestyle.abbott/ca-en/freestyle-libre-2.html" target="_blank" data-msys-clicktrack="0" rel="nofollow noopener noreferrer">https://www.freestyle.abbott/ca-en/freestyle-libre-2.html</a></li></ul><p>(2) You must be 18 years of age or older, or be accompanied by a parent, guardian or caregiver who is 18 years of age or older, to attend this event.</p><p>(3) Sensors are available for eligible patients who meet the labelling requirements of 4 years or older with diabetes mellitus. Limit: one per registrant.</p><p>(4) The FreeStyle Libre 2 flash glucose monitoring system is indicated for measuring interstitial fluid glucose levels in people aged 4 years and older with diabetes mellitus.
