In this session, you will learn about your healthcare rights, how to prepare your family and friends, and how to plan for the care you want—now and in the future.
Topics include:
- Identifying your Substitute Decision Maker (SDM)
- Understanding how Ontario law governs consent to care
- Appointing a Power of Attorney for Personal Care, if needed
- Sharing your values, wishes, and beliefs to guide future care decisions
