Who? Holland Bloorview youth 14 years and older and their families.What?<ul><li>Meet directly Holland Bloorview's Bridging to Adulthood team.</li><li>Receive 1:1 support or coaching from our staff and/or healthcare students to answer any question you may have about the transition to adulthood.</li><li>Explore what resources could help your immediate needs, or align with your future goals.</li><li>Sign up for other resources and/or supports if needed.</li></ul>Where? Virtually through Zoom Healthcare*however we can meet with you over the phone or on-site at Holland Bloorview as well.When? Tuesday June 10th, 2025 - 2 timeslots12:00 - 1:00pm2:00pm -3:00pmWhy? Have you started your transition to adulthood journey and now have a bunch of questions you need answered? Are you unsure what funding supports you are eligible for? Do you need support completeing an application?
