Work through your own plan and receive coaching and support as you go! Where? On-Site at Holland Bloorview Kids Rehabilitation Hospital When? Monday April 17th, 20236:00pm-7:30pmWhy? Transitioning to adulthood has many different moving parts. Come learn and make your own plan to make the transition as smooth as possible.
