Who? Holland Bloorview youth with disabilities (15+) and their families.What? Come on-site and work with Transition Team members to develop your own Personalized Transition Plan to adulthood. Work through your own plan and receive coaching and support as you go! Where? On-Site at Holland Bloorview Kids Rehabilitation Hospital When? Monday September 18th, 20236:00pm-7:30pmWhy? Transitioning to adulthood has many different moving parts. Come learn and make your own plan to make the transition as smooth as possible.
