When? Wednesday January 11th, 2022 from 12:00-1:00pm

Why? Many children have been receiving Primary Care from their developmental Pediatrician. In Ontario, many Pediatrics centers discharge the child at their 18th birthday. Primary Care (i.e. Family Doctor or Nurse Practitioner) helps you manage new (non-emergency) or on-going conditions. Primary Care is important to sort as the child transitions to adulthood to support with areas such as:

-Ontario Disability Support Program (ODSP) medical paperwork

-Support managing chronic conditions

-Prescriptions

-Routine screenings and check-ups

-And more!
