About this event

This is a 6 week course on Wednesday afternoons, April 17 to May 22, 2024, from 1:30 to 3:00pm EST. Due to funding, priority will be given to Southeast Ontario residents.


Powerful Tools for Caregivers is an educational program designed to help family caregivers take care of themselves while caring for a relative or friend. You will benefit from this class whether you are helping a parent, spouse, friend, someone who lives at home, in a nursing home, or across the country. By taking care of your own health and well-being, you can become a better caregiver.

Class participants are given The Caregiver Helpbook to accompany the class and provide additional caregiver resources.

The Self-Management Program of Southeastern Ontario is funded by the Ontario Ministry of Health and Long-Term Care to coordinate, promote and facilitate self-management workshops across Southeastern Ontario.

The Self-Management Program of Southeastern Ontario coordinates self-management workshops, providing the opportunity for individuals, caregivers and health care providers to learn about self-management and support for people living with (or at-risk of developing) chronic conditions.

Our program goal is to provide individuals with the knowledge, skills and confidence to self-manage their chronic conditions and to provide health care professionals with the tools and techniques to offer better self-management support. 

The anticipated impact is that individuals will have a stronger ability to self-manage their chronic conditions and live healthier lives, thereby improving health outcomes and reducing long-term health care costs.  This centralized coordination helps promote access to self-management supports, programs and resources throughout the community. 

The Self-Management Program of Southeastern Ontario is housed at the Kingston Community Health Centres.
